Sign and date your application

• Complete and sign the disclosure form

Obtain Your PA Mandated Criminal Background Check

Free for volunteers.

Criminal Background Check

Obtain your PA mandated Child Abuse Background Check

Free for volunteers.

Child Abuse Background Check

Do one of the following

• If you have lived in the state of PA for the last 10 years download and complete the Volunteer Disclosure Form PA Act 153

• If in the last 10 years you have lived outside of PA, you must complete the Federal Bureau of Investigation Criminal Background Check and get fingerprinted.
