• Initial 4xs above the signature line
• Sign and date your application
• Complete and sign the disclosure form
• Unit leaders’ signatures

Child Abuse Background CheckDo one of the following
• If you have lived in the state of PA for the last 10 years download and complete the Volunteer Disclosure Form PA Act 153

• If in the last 10 years you have lived outside of PA, you must complete the Federal Bureau of Investigation Criminal Background Check and get fingerprinted.
